Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction.
- Maintain clear, concise, accurate, and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals.
- Design functional treatment programs that support effective patient return to community activities and progress patients toward discharge within an established length of stay.
- Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations.
- Adhere to discipline-specific State Practice Act and regulatory requirements, maintaining professional licensure in good standing and completing all mandated continuing education requirements.
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of Texas.
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
- Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
- Knowledge of and adherence to discipline-specific State Practice Act and professional code of conduct.
- Commitment to patient and employee safety practices including safe patient handling and infection control protocols.
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting.
- Experience supervising or mentoring physical therapy students.
- Active involvement in continuing education and professional development activities.
