Physical Therapist – Home Health

Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide skilled physical therapy services to home health patients in the East Portland area and surrounding regions
- Conduct patient evaluations and develop individualized treatment plans in a 1:1 care setting, visiting up to 6 patients per day
- Complete OASIS assessments and maintain accurate clinical documentation using EPIC Remote Client and Rover mobile application
- Manage daily patient scheduling and travel efficiently within an assigned geographic service area
- Collaborate with the care team to ensure continuity and quality of patient care in the home health setting
Required Qualifications
- Active Physical Therapist license in Oregon (pending license considered)
- Minimum 1 year of skilled home health experience
- OASIS experience required
- Basic Cardiac Life Support (BCLS) certification required
- Must have reliable personal transportation; daily travel up to approximately 60 miles may be required
Preferred Experience / Skills
- Familiarity with EPIC Remote Client for home health charting
- Experience using mobile clinical applications such as Rover
- Strong time management skills with the ability to independently manage a daily patient visit schedule
