Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in collaboration with the interdisciplinary team and in accordance with physician direction.
- Manage individual, concurrent, and group treatment sessions; coordinate discharge planning including equipment trialing, home exercise program instruction, and arrangement of continuing services.
- Complete all medical record documentation accurately and in a timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, and discharge documentation in compliance with state, federal, and payer guidelines.
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring the plan of care is communicated and progressed throughout the length of stay.
- Supervise and mentor therapy students as assigned, including development of learning objectives and completion of student evaluations.
- Participate in performance improvement initiatives and maintain awareness of organizational safety standards, including safe patient handling, infection control, and workplace hazard reporting.
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of Arizona.
- Adherence to the state Physical Therapy Practice Act and all applicable regulatory requirements.
- Completion of all state-mandated continuing education requirements to maintain licensure in good standing.
- Demonstrated ability to perform comprehensive patient evaluations and develop clinically reasoned treatment plans.
- Commitment to patient and employee safety standards and organizational compliance policies.
Preferred Experience / Skills
- Experience in an inpatient rehabilitation or acute care hospital setting.
- Prior experience supervising Physical Therapy students or clinical affiliates.
- Active involvement in professional organizations or continuing education beyond minimum licensure requirements.
