Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Minnesota Physical Therapist license is required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum 6 months of recent experience in a skilled nursing facility (SNF) or similar post-acute setting.
- Experience managing daily patient volumes consistent with SNF expectations, including productivity standards.
Preferred Qualifications
- Experience with high daily patient volumes (approximately 8–15 patients per day).
- Familiarity with productivity standards and documentation expectations in long-term care or senior living environments.
- Previous travel or contract therapy experience.
Assignment Details
- Guaranteed hours: 36 hours per week with an 85% productivity standard.
- Schedule: Monday through Friday, five days per week.
- Setting: Skilled nursing / senior living facilities in Spring Valley area; candidate may be required to float between nearby sites as needed.
- No call or weekend coverage required.
- Typical daily volumes: 8–15 patients.

Impacting Patient Care
Provide rehabilitative services to older adults in skilled nursing and senior living communities. Support continuity of care by delivering evaluations, individualized treatment plans, and progress documentation.
