POSITION DESCRIPTION
TITLE: Physician/Medical Director DEPARTMENT: Health Services
RESPONSIBLE TO: Director of Health Services/Nursing
RESPONSIBLE FOR: Dotted line to Nurse Practitioners and other clinic health professionals
JOB SUMMARY:
As Physician, they will provide medical care and participate in the resident’s care plan by providing community with pertinent information regarding past medical history, medication and treatment orders, diet orders, and orders for routine care to maintain or improve functional abilities.
As Medical Director, they are responsible for coordinating medical care and helping to implement and evaluate resident care policies that reflect professional standards of practice.
REQUIRED EDUCATION, EXPERIENCE, SKILLS:
1. Appropriate Medical degree from an accredited college or university.
2. Board certification in family medicine, emergency care, or occupational health.
3. License and certification to practice medicine.
4. 10 or more years’ experience in clinical medicine required.
5. 5 or more years’ experience is preferred in hospital administration.
6. Must have good assessment and evaluation skills, supervisory experience, good communication skills, good interpersonal skills, and problem identification and solving skills.
JOB REQUIREMENTS:

3. Must comply with Code HIPAA-related Policies and Procedures, Infection Prevention procedures, Resident Rights guidelines.
4. Must comply as a mandated reporter to report any act or suspicion of Abuse to my supervisor immediately and/or as required by the Elder Justice Act.
5. Employment and annual tuberculosis testing as required by Virginia State Licensure.
6. Must attend mandatory in services required by the State Licensure and all other mandatory in services required by regulatory agencies and/or by SVWC.
7. Follows and supports the policies and procedures established by SVWC.
ESSENTIAL DUTIES/RESPONSIBILITIES - PHYSICIAN:
1. Provide visits for residents, provide medical orders, and review a resident’s medical condition as required.
2. Keep residents and/or responsible parties informed of the resident’s medical condition and respond to resident/family inquiries in a timely manner.
3. Maintain internal services provided under Medicare/Medicaid contract and not order lab or other services from providers in which Physician, or Physician’s family, has a vested interest, in violation of state or federal law.
4. Provide for care of residents with complex or special needs, such as dialysis, hospice or end-of-life care, intravenous medications/fluids, dementia and/or related conditions, or problematic behaviors or complex mood disorders.
5. Provide both primary care services and urgent care services for employees of the Community. Such services should be limited to ordinary business hours and should not include on-call coverage.
6. Review of requested resident’s overall condition and program of care at each visit, including medications and treatments; ensuring adequate frequency of visits.
7. Signing and dating, within regulatory requirements of all orders, such as medications, admission orders, and re-admission orders; and documentation of progress notes with signatures.
8. Review, respond to, and participate in federal, state, local, and other external surveys and inspections. To that end, Community shall notify Physician of any such survey or inspection as soon as practicable. The Physician shall be familiar with of all quality of care and medical issues noted during the survey or inspection. The Physician shall provide input on any plan of correction or in any dispute resolution resulting from a survey or inspection and participate in the implementation of such plan.
ESSENTIAL DUTIES/RESPONSIBILITIES – MEDICAL DIRECTOR:
1. Oversee the integrated delivery of care services, such as medical, nursing, pharmacy, social, rehabilitative and dietary services, which include clinical assessments, analysis of assessment findings, care planning including preventive care, care plan monitoring and modification, infection control (including isolation or special care), transfers to other settings, and discharge planning.
2. Recommending, developing, and approving admission policies and care practices that address the types of residents that may be admitted and retained based upon the ability of the Community to provide the services and care to meet their needs.
3. Approve availability, qualifications, and clinical functions of staff necessary to meet resident care needs.
4. Manage resident formulation of advanced directives (in accordance with State law) and Community implementation of policies and procedures to honor Resident advance directives, including those addressing end-of-life care.
5. Monitor provisions that enhance resident decision-making, including choice regarding medical care options.
6. Conduct research, if allowed by the Community and in accordance with all applicable laws, regulations and ethical standards with respect to research, within the Community.
7. Provide medical guidance of rehabilitation services as applicable, including physical therapy, occupational therapy, speech therapy, respiratory therapy, and mental health programs.
8. Ensure systems are in place for licensed practitioners (e.g., nurse practitioners) that perform physician-delegated tasks within the regulatory requirements and within the scope of practice as defined by State law and any applicable professional practice standards.
9. Procedures and general clinical guidance for Community staff regarding when to contact a practitioner, including information that should be gathered prior to contacting the practitioner regarding a clinical issue/question or change in condition.
10. Physician shall review, respond to, and participate in federal, state, local, and other external surveys and inspections. ESSENTIAL DUTIES/RESPONSIBILITIES – COMBINED (PHYSICIAN/MEDICAL DIRECTOR):
1. Medical Director helps the Community obtain and maintain timely and appropriate medical care that supports the healthcare needs of the residents, is consistent with current standards of practice, and helps the Community meet its regulatory requirements. Physician addresses issues related to the coordination of medical care identified through the Community’s quality assessment performance improvement committee and quality assurance program, and other activities related to the coordination of care, which may include, but is not limited to helping the Community:
a. Ensure that residents have primary attending and backup physician coverage.
b. Ensure that Community’s resident health clinic is staffed during reasonable operating hours, as established in collaboration with Community.
c. Ensure that physician services are available 24 hours a day and in case of emergency.
d. Ensure health care practitioner services are available to help residents attain and maintain their highest practicable level of functioning, consistent with current standards of practice and regulatory requirements.
e. Develop a process to review basic physician and health care practitioner credentials (e.g., licensure and pertinent background).
f. Resolve concerns and issues between the Physician, health care practitioners and Community staff.
g. Help foster relationships in the greater medical community for supportive and consultative needs, including but not limited to, hospice, occupational/employee health, hospital system, other long term care providers and infection control.
h. Resolve issues related to continuity of care and transfer of medical information between the Community and other care settings.
i. Facilitate feedback to other health care practitioners about their performance and practices.
j. Review individual resident cases as warranted to evaluate complaints, concerns, or other problematic situations and take appropriate steps to resolve the situation as necessary and as requested.
k. Discuss and intervene (as appropriate) with a health care practitioner about medical care that is or may be inconsistent with applicable current standards of practice or stated preferences of the resident.
l. Review consultant recommendations that affect Community’s resident care policies and procedures or the care of individual resident.
m. Address concerns between residents’ attending physician and the Community.
n. Identify Community or practitioner educational and informational needs, and provide information to the Community practitioners from sources such as nationally recognized medical care societies and organizations where current clinical information can be obtained; and
o. Help educate and provide information about resident care to Community staff, practitioners, residents, families and others.
p. Collaborate with one or more nurse practitioners who are employees or contractors of the Community, such supervision will be in accordance with appropriate protocols and laws and regulations of the Commonwealth of Virginia.
q. Consult and cooperate with Community regarding all risk management programs, quality improvement activities, and in the defense of all claims.
2. Courteous and friendly attitude towards residents, visitors, and fellow employees. Accepts and asks questions of appropriate individuals. Maintains a cheerful disposition, even when experiencing difficulties. Accepts and asks questions of appropriate individuals.
3. Ability to deal with a wide variety of personalities, especially in elderly people. Responds appropriately to people experiencing confusion, anger, or unhappiness, as well to those who may be abusive. Knowledgeable of Residents Rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all clients in a safe, secure environment.
4. Always provide the highest quality of service, strive for accuracy, completeness, and neatness in all endeavors. Demonstrates an ability to accomplish assigned tasks in a timely manner. Ability to remain flexible and adjust priorities as needed.
5. Availability may include evenings, weekends, and holidays (department dependent). If needed, must be easily reached by telephone.

Category D: The person holding this position understands Universal Precautions Risk Classification categories apply to this position and the individual may be exposed to AIDS, HIV, and Hepatitis B viruses. Tasks that involve no exposure to Blood, Body Fluids, or Tissues. The normal work routine involves no exposure to blood, body fluids, or tissues (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids).
