Role Description

This is a full-time role as a Home Health Physical Therapist based in South Dallas, TX. The role involves assessing patients' physical therapy needs, developing and implementing tailored treatment plans, and providing hands-on care in the home setting. You will work collaboratively with multidisciplinary teams, monitor patient progress, and provide education to patients and their families to promote optimal recovery and independence.

Qualifications

• Expertise in Physical Therapy and Rehabilitation techniques for diverse patient needs
• Strong knowledge of Home Care best practices
• Proven ability to deliver high-quality Patient Care with empathy and professionalism
• Experience working in home health settings or providing individualized care, preferred
• Valid Physical Therapist license in the state of Texas
• Effective communication and collaboration skills with patients, families, and care teams
• Proficiency in documentation and adherence to patient care protocols
• Reliable transportation and willingness to travel within the South Dallas area
