Position Summary
We are seeking a compassionate and motivated Physical Therapist to join our senior rehabilitation team. The Physical Therapist will evaluate, develop, and implement individualized treatment plans for geriatric patients recovering from surgery, illness, injury, or functional decline. We offer relocation assistance for qualified candidates and new graduates with clinical experience are encouraged to apply.

- Competitive salary: $95,000–$120,000 annually

Responsibilities
- Perform comprehensive physical therapy evaluations.
- Develop and implement individualized treatment plans.
- Provide therapeutic exercises, gait training, balance training, and fall-prevention programs.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and family members.
- Maintain accurate clinical documentation and compliance with regulatory requirements.
- Educate patients and caregivers on home exercise programs and safety strategies.
- Participate in care planning and interdisciplinary team meetings.

Requirements
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent degree.
- Current state Physical Therapist license or eligibility for licensure.
- Experience in skilled nursing, post-acute rehabilitation, home health, or geriatric care preferred.
- Strong communication and teamwork skills.
