Sauk Prairie Memorial Hospital & Clinics has continually challenged itself to surpass the expectations of a small rural hospital. Through careful planning, physician recruitment, investments in technology and good medicine, SPMHC has gained a regional reputation for excellence in orthopedics, general surgery, spine surgery and gynecologic surgery.

More than 2,200 inpatients are discharged annually and patient revenue exceeds $50 million. Approximately 34 physicians serve as full-time active members of the medical staff, while more than 80 physicians offer consulting services.

The hospital complex is just blocks from the majestic Wisconsin River where bald eagles feed in the open waters near the Prairie du Sac dam. Summer days can be spent walking along the river path, or climbing the bluffs of Devil's Lake State Park, just 15 minutes from town. This beautiful scenic community is located just 30 minutes from Madison, which offers an array of shopping and arts, as well as a world class university.

Company History

Founded in 1956, SPMHC includes a 36-bed, not-for-profit (501c3) acute care hospital located in Prairie du Sac, Wisconsin and four primary care clinics in Black Earth, Plain, Spring Green and Lodi. SPMHC provides care for more than 40,000 people in our service area, which extends from Poynette to Lone Rock, from Plain to Black Earth.

Benefits

Paid Time Off
This benefit offers a bank of time to include vacation, sick and holiday time. Paid time off accrual begins on the first date of employment. Employees may access the paid time off bank immediately after hours are accrued. Accrual of paid time off hours is calculated on hours worked in a 40-hour work week. Accrual continues until reaching a maximum of 1.5 times the annual accrual amount.

Retirement Plan
The benefit allows employees to contribute a percentage of income into a retirement plan beginning on the first date of employment. After 90 days of benefit-eligible employment, SPMHC provides a matching contribution if the employee contribution is at least 3%. This plan offers immediate vesting.

Health Insurance
Eligible the first of the month following 30 days of employment.
SPMHC provides a comprehensive health plan which is self insured utilizing the Dean TPA network of providers. This is a $250/$500 deductible/co-insurance plan and provides 100% coverage for annual exams and well child check ups, including immunizations. Vision benefits are included subject to deductible/co-insurance. Prescription coverage provided by Navitus as part of the health plan. Dependents until age 25 (student status not required) and domestic partners included.

Wellness Center
Free single membership to our on-site fitness facility. Couple and family memberships are discounted.

Dental Insurance
Eligible the first of the month following 30 days of employment.
SPMHC provides dental benefits through Delta Dental PPO. Diagnostic and preventative services covered at 100%. Other covered services subject to a $25 deductible and co-insurance.

Life Insurance
Eligible the first of the month following 30 days of employment.
Employer-paid benefit provides group term life insurance to include accidental death and dismemberment. Elective employee-paid life insurance may be purchased on self, spouse, and/or dependents at group rates.

Disability Insurance
Eligible the first of the month following 30 days of employment.
Employer-paid short-term disability provides income protection to an employee for up through 11 weeks. Elective employee-paid long-term disability may be purchased to extend benefits beyond 11 weeks.

Flexible Spending Account
Eligible the first of the month following 30 days of employment.
This allows for employees to pay for health and dental insurance premiums, dependent care expenses, and medical expenses on a pre-tax basis as per IRS guidelines.

Tuition Reimbursement
Eligible after 6 months of benefit-eligible employment.$2,500 per calendar year as per the tuition reimbursement policy.

Employee Assistance Program
Assistant to employees with personal circumstances which could be helped through counseling.
