Position Overview:
We are looking for a compassionate, dedicated, and experienced Primary Care Physician (Internal Medicine or Family Medicine) to join our dynamic team. As a Primary Care Physician at Foundation Medical Partners, you will play a key role in diagnosing, treating, and providing preventive care for patients. You will benefit from our connected care model, which gives you access to a wide range of specialties, enabling you to provide coordinated and comprehensive care to our diverse patient population.
Responsibilities:
- Provide comprehensive primary care to patients of all ages (family medicine) or adults (internal medicine), including preventive care, diagnosis, treatment, and management of acute and chronic conditions.
- Perform physical examinations, take medical histories, and order necessary tests to guide treatment plans.
- Diagnose and treat various conditions, creating individualized care plans for each patient.
- Collaborate closely with specialists across a broad range of disciplines within our connected care model to provide comprehensive, coordinated care.
- Educate patients and their families on wellness, lifestyle modifications, and disease prevention.
- Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
- Participate in community outreach programs and health education initiatives.
- Foster a positive and supportive work environment, promoting teamwork, communication, and professional development.
Qualifications:
- Medical degree (MD or DO) from an accredited medical school.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Current, unrestricted medical license in NH.
- BLS/ACLS certification required.
- Strong clinical skills with the ability to manage a wide variety of conditions.
- Excellent communication skills and the ability to build strong relationships with patients and colleagues.

- Ability to work collaboratively within a connected care model, integrating input from specialists to deliver comprehensive care.
