Job Summary: This position is a hybrid role blending clinical support at our Adult Day Center with professional care coordination. The Associate Care Manager (AGCM) assists the care management team in providing ongoing oversight of care to older adults, supports families, and assures that resources are appropriately matched to meet the client’s ongoing needs.
Work Schedule
In-Center Clinical Day: Tuesday, 9:00 AM to 3:30 PM (Required).
Field & Office Hours: Flexible hours (Monday, Wednesday, Thursday, Friday) as needed for field supervisory visits and coordination tasks.
On-Call: Rotates weekend call time.
Role 1: Associate Care Manager (ECC - Field/Office)
Care Coordination: Take direction from the geriatric care management team on tasks that assist coordinating care, such as calling medical providers for appointments, pharmacy medication boxes, and durable medical equipment.
Client Monitoring: Provide oversight to stable, chronic individuals.
Family Support: Educate families as related to the Geriatric Care Managers care plan.
Documentation: Provide notations in the computer for review by an immediate supervisor. The AGCM is responsible for billing all of their documentation as part of the charting process.
Transitions: Support transitions from one level of care to another, including short hospitalizations or permanent moves.
Community Resources: Access community resources and perform web-based searches.
Role 2: Adult Day Center Nurse (Senior Moments - In Center)
Clinical Monitoring: Monitor wellness indicators including blood pressure, weight, and blood sugar.
Medications: Administer medications per policy and in regard to all medication regulations.
Direct Care: Assist clients to rise from a seated position, assist with walking, and transfer clients in their chairs when they are unable to assist.
Program Support: Orient all staff to health-related needs of each participant and assist the coordinator of recreational services with specific activities that promote health.
Requirements
Physical Requirements (Strict Adherence)
No Light Duty: There is no light duty at Senior Moments. Employees must assist with physical care.
Lifting & Transfers: Must be physically fit enough to lift walkers and/or wheelchairs from a car and help the client transfer in and out of the car.
Mobility: Ability to bend and stoop to pick up items from the floor (10 lbs or less) and walk up and down stairs to retrieve supplies.
Driving: Must be physically fit enough to drive to visit several clients during the same day.
Job Requirements
Licensure: Current LPN License.
Experience: Experience with geriatric population required.
Professionalism: Expected to maintain a high level of independence and professional conduct.
Transport: The employee will be required to transport people to appointments.
