Seven Healthcare are seeking a dedicated Physical Therapist for a travel assignment in New York. This role involves working within a team environment, providing comprehensive physical therapy services in patients' homes. The successful candidate will support the Monroe County team exclusively, ensuring high-quality patient care and contributing to a collaborative healthcare environment.

Key Responsibilities of the Physical Therapist

• Provide case management and direct patient care within the team environment
• Administer comprehensive physical therapy modalities in the home setting
• Assist the physician with evaluating level of function and developing the plan of care
• Prepare and maintain clinical and progress notes
• Consult with and educate patients, families, and team members
• Develop and revise treatment and discharge plans
• Supervise Physical Therapy Assistants to ensure patient health, safety, and care compliance
• Periodically evaluate patient's progress and adjust therapy program as needed
• Complete all clinical records for compliance with state and federal regulations

Requirements of the Physical Therapist

• A B.S., M.S., or D.P.T. degree in Physical Therapy
• Possession of a current New York State License in Physical Therapy
• BLS (AHA only)
• Oasis experience
• Minimum of one year experience in Long Term Care/Home Care setting
• Recent Home Care experience within the last year
