Position Highlights & Primary Responsibilities:
- Provide daily rounding on patients in skilled nursing facilities (SNF) and long term care (LTC) settings.
- Serve as the primary provider for one to two SNFs, managing patients across both skilled and long term care levels.
- Complete an average of 6-9 patient encounters per day, depending on patient complexity, acuity, and the number of facilities covered. The typical combined daily skilled census across one to two SNFs ranges from 8-13 patients.
- Participate actively in weekly interdisciplinary team meetings at the SNF, as well as departmental and multi specialty meetings.
- Collaborate closely with Extended Care physicians and the patient's primary care team to align care with patient goals and deliver high quality, value based care.

Required Qualifications:
- Current Washington state license, or ability to obtain prior to hire
- Nurse Practitioner Certification (NP C) or Physician Assistant (PA C) certification, or ability to obtain before hire
- Strong communication skills, a record of high patient satisfaction, and demonstrated ability to promote effective teamwork
Preferred Qualifications:
- Education or work experience in SNF/long-term care or hospitalist medicine is ideal.
- Experience or formal training in geriatrics, complex care management, or palliative care.
- Acute care certification or a minimum of 1 year of clinical experience.
- Demonstrated skill in working with a multi-disciplinary team
Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
